Help give a child the opportunity to experience the freedom, independence, and joy of adaptive cycling.
Complete the application below to be considered for Project Mobility's Adaptive Bike Giveaway Sweepstakes.
How It Works
Step 1
Submit an online application.

Step 2
If approved, your child will participate in the next Adaptive Bike Giveaway Sweepstakes campaign.
Step 3
Share your child's campaign with family, friends, and your community. Supporters may donate to your child's campaign, with every donation earning entries according to the Official Entry Schedule.
Step 4
Raise at least $3,000 during the campaign period to qualify for the certified random drawing.
Step 5
At the end of the campaign, all qualified participants are entered into a certified random drawing.
MEETING OR EXCEEDING THE FUNDRAISING GOAL DOES NOT GUARANTEE RECEIPT OF AN ADAPTIVE BIKE.
Every qualified participant has a chance to win, with the odds determined by the number of eligible entries earned during the campaign.

Eligibility & Participation Requirements
To be eligible for the Adaptive Bike Giveaway Sweepstakes, applicants must meet all of the following requirements:

Child must be 21 years of age or younger at the start of the campaign.

Child must reside in or near the Chicagoland area.

Complete an in-person evaluation and adaptive bike fitting in St. Charles, Illinois within 7 days of application approval.

Provide transportation to all required appointments.

Agree that Project Mobility may photograph and record your child and family and use photos, videos, names, stories, and other submitted content on its website, social media channels, marketing materials, and other promotional media.

Attend the adaptive bike presentation at the date, time, and location designated by Project Mobility.

Comply with all program guidelines, deadlines, and participation requirements.
Failure to meet any of the above requirements may result in removal from the campaign, forfeiture of the adaptive bike, and/or ineligibility for future Project Mobility bike giveaway programs.

Submitting an application does not guarantee acceptance into a campaign.

Approval to participate does not guarantee receipt of an adaptive bike.

MEETING OR EXCEEDING THE FUNDRAISING GOAL QUALIFIES A PARTICIPANT FOR THE CERTIFIED RANDOM DRAWING BUT DOES NOT GUARANTEE RECEIPT OF AN ADAPTIVE BIKE.Winners are selected through a certified random drawing in accordance with the Official Rules.

All funds raised remain with Project Mobility to support its mission of providing adaptive bikes, adaptive equipment, inclusive programs, and other mobility initiatives that create freedom, independence, and joy for individuals with disabilities, regardless of the outcome of the certified random drawing.

Adaptive bikes are custom-built and delivery times may vary.

Additional adaptive bikes may be awarded if campaign fundraising milestones are reached.​

Before submitting your application, please carefully review the following. By signing and submitting this application, you acknowledge that you have read, understand, and agree to everything in this application, the participation requirements, and the Official Rules of the Adaptive Bike Giveaway Sweepstakes.

I understand and agree that:

✔ THIS IS A CERTIFIED RANDOM DRAWING. No matter how much money my child raises, they will receive an adaptive bike through this sweepstakes ONLY IF THEY ARE SELECTED IN THE CERTIFIED RANDOM DRAWING.
✔ MY CHILD MUST RAISE A MINIMUM OF $3,000 DURING THE CAMPAIGN PERIOD TO QUALIFY FOR THE CERTIFIED RANDOM DRAWING. If my child does not raise at least $3,000, they will NOT be eligible for that campaign's certified random drawing.

✔ MEETING OR EXCEEDING THE FUNDRAISING GOAL QUALIFIES MY CHILD FOR THE CERTIFIED RANDOM DRAWING BUT DOES NOT GUARANTEE RECEIPT OF AN ADAPTIVE BIKE.
✔ ANY FUNDS RAISED REMAIN WITH PROJECT MOBILITY to support its mission of providing adaptive bikes, adaptive equipment, inclusive programs, and other mobility initiatives. Funds are NOT REFUNDABLE, regardless of the outcome of the certified random drawing.

✔ MY CHILD AND FAMILY AGREE TO ATTEND ALL REQUIRED EVALUATIONS, BIKE FITTINGS, AND THE ADAPTIVE BIKE PRESENTATION at the date, time, and location designated by Project Mobility. Failure to attend any required appointment or the adaptive bike presentation MAY RESULT IN FORFEITURE OF THE ADAPTIVE BIKE AND INELIGIBILITY FOR FUTURE PROJECT MOBILITY BIKE GIVEAWAY PROGRAMS.
